Location: Urban (Rochester NY), with ~750,000 inhabitants in the region.

Staffing:
One MD at 0.25 clinical FTE (the rest of the time I’m teaching with the IHI, leading the IMP Project, etc).
One RN part time (also faculty on the IMP project).

Patient panel size: ~400

Access:
24/7/365 via answering machine and cell after hours. We share occasional backup with other IMPs in our region.

Space:
Two offices about 125 sq ft each (one for Judy, one for Gordon).
One exam room about 120 sq feet.
Shared waiting room (with Dr. Lee) with three chairs and an old laptop so folks can go on line and take the HowsYourHealth survey.
